Membership Cancellation

If you are a current member and need to fill out or Membership Cancellation form online, please click the link below. If for any reason you decide to cancel your membership, please note you will be charged another month's tuition as your exit fee. We will only waive exit fees for serious injuries or if you are moving out of state/the country and will no longer be able to train. If you have any questions regarding membership, please reach out prior to completing the cancellation form.
